Deeming

Northeast Florida Health Services, Inc. d.b.a. Family Health Source is a health center program grantee under 42 U. S. C. 254b, and a deemed Public Health Service employee under 42 U. S. C. 233(g)-(n)

Family Health Source is a Federally Qualified Health Center (FQHC) and a Federal Tort Claims Act (FTCA) Deemed Facility, covered by professional liability insurance through our enrollment in the Federal Tort Claims Act program. For further information, please contact our Administration office at 386-202-6025.

This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims for itself and its covered individuals.
